Online Signature
Online signature acceptance is now available for the membership waiver, code of conduct, background screening and the Junior Club Personnel Code of Ethics. Adult Members and Parents/Guardians of Junior members will need their drivers license number in order to be able to complete the online signature process.
2010-11 Updates
Effective September 1st, you may start the application process for your 2010/2011 season membership. Any adults associated with a junior program and/or officiating or directing at junior events needing a background screening (renewal or new) needs to register & pay 1 to 2 weeks prior to participating so that their background screening can be completed.
All participants (or parent/guardian) need to do their own registration so that the membership waiver acceptance is legally binding. The membership waiver acceptance requires the Adult or Parent/Guardian of the junior member to enter their Driver’s License number or last 4 digits of their SS#. So have the license on hand before starting the process. You can pay online with a Visa or MasterCard as you complete the registration. You will need a card with a 3 digit security code on the back.
ALL ADULTS affiliated with a junior program or working junior events must sign a USA Volleyball Junior Club Personnel Code of Ethics. Electronic acceptance of this form will be included as you complete your online registration. The acceptance is initiated once you indicate you will be working with a junior club or officiating or directing at junior events. Junior age coaches will need to complete a hard copy of the Personnel Code of Ethics signed by them as well as their parent/guardian and mail or FAX it to the Region Office. (The form can be found on the region website.)
You must have a current membership prior to participating in any sanctioned event (including tryouts). A current membership includes having registered, paid and have a current background screening when required. If you choose not to pay online (Submit payment separately) you need to send payment including member name and date of birth to the Region Office so your membership can be activated prior to participating.

Online Registration Reminders
Be aware that with electronic acceptance it is important that adult members register themselves and that a parent/guardian registers a junior player so that the acceptance is legally binding.
With the Online signature, the individual completing the registration for a junior player is representing that he/she has the legal capacity and authority to act for and on behalf of the minor.
Coaches and Club Directors should not be registering their players. If the Club Director/Team Rep has access to a computer lab they might have a parent computer registration night for those unable to register on their own.
To register online without paying by credit card you simply need to click on Submit Payment separately button. The registration will then go into the system unpaid. Then you can submit payment to your Club Director/Team Rep or the Iowa Region Office directly. To activate the Electronic Signatures when you are online you need to open and close the links to the membership forms (Waiver & Release of Liability, Code of Conduct & Junior Club Personnel Code of Ethics, if required) and then click the acceptance boxes to complete the registration process.
The Adult registrant or the Parent/Guardian for a junior age registrant will be asked for their Driver’s License or last 4 digits of SS # number to complete the electronic signature acceptance. So have the license on hand before starting the process.
The background screening is initiated when an adult (18 years old) indicates they will be associated with a junior program or working junior events. If an individual is prompted to do the background screening they must ‘Continue & Process Background Screening’ to complete the registration. (Otherwise, they will have to start the registration process over.) To complete the screening after becoming an active member they just login and click on the Background Screening Authorization tab to complete the process.
If you are unable to complete the online process you will need to do a paper individual membership form. The applicant and parent/guardian for a junior age applicant will need to sign the form and send the first page to their Club Director/Team Rep or the Iowa Region Office. Any other required forms will also need to be completed in paper form and submitted. Credit Cards
1. Only Master Card or Visa are accepted and must have a 3 digit security code on the back to be used online. 2. To pay by MasterCard or Visa after registering you can login and use the 'To Pay For Your Membership Online Click Here' link or call the Region Office at 515-727-1860 to have staff process the card for you. |
