Team Reps should send in the following information to the Iowa Region Office:

• Team Application
• Signed Waiver & Release of Liability for all players and staff that do not register on line.
• Signed Background Screening forms for all Junior team staff that need to go through the process this season (good for 2 years and part of the online registration process). This form is needed for all Coaches or Assistant Coaches 18 years of age or older.
• Signed Code of Ethics for all Junior Team Coaches and Assistant Coaches.
• Signed Chaperone Responsibility for all team chaperones. (Chaperones are required at Qualifiers and National events)
• All Fees Owed (Individuals may pay on line or with the team). Club Directors or Team Reps should go on line and print their Club Unpaid list and check mark the members they are for whom they are paying.
