FIRST - Read the paragraph below that applies to you.
SECOND - Open and/or print out the Screen Shots provided by the Region to assist you during the registration process. These are viewable in Adobe Reader.
THIRD – To complete the online registration with online signature you will need to have your driver’s license number. Junior age players will need to have a parent complete the process, and the parent will need their driver’s license number.
FOURTH – Membership fees may be paid online with a MasterCard or Visa. If you do not pay online, send your money and a copy of the confirmation to the Region Office or give to your Team Rep to send in as a group with other team members’ forms.
FIFTH – Begin registration by clicking one of the Login buttons above.
SIXTH – If you do not know your password you will need to have your user name or email address that is already on the online system and your date of birth.
SEVENTH – Junior Team Staff will need to sign a Coaches Code of Ethics and Background Screening form (every other year) and Chaperone Responsibility. Mail signed forms to: Iowa Region, 10861 Douglas Avenue, Suite C, Urbandale, IA 50322.
If you are a Junior Player
Your parent/guardian will need to complete the online membership process for you. They will need their driver’s license to complete the process.
For those members signing up for Tryouts, choose UNDECIDED as the club. When you are selected and accept you may then go back and change to the club you are joining. When you finish the Registration process print the confirmation page and take a copy to all Tryouts and give to the club you choose. You must also print and fill out the Junior Medical Release Form (part of online registration process) and give this to your Coach. (Players should keep a copy with them at all times.)
If you are a Coach, a Team Rep, or Club Director
You will need your driver’s license to complete the process. When you finish the Registration process, print the confirmation page and give to your club director. Every other year, coaches, team reps, and directors must completed and sign the online Background Screening form (good for 2 years and part of the online registration process). Note: The background screening process can not begin until your signed form and membership fees are in the Regional Office. All coaches need to sign the Coach's Code of Ethics form and send to the Region Office. You will need to also do an Adult Medical Release form that you should carry to events.
Junior Age Assistant coaches that are 18 years old or will turn 18 years of age need to do the background screening process. If you did not pay online send your money and a copy of the confirmation to the Region Office or give to your Team Rep to send in as a group with other team members’ forms.
If you are a Chaperone
You will need you driver’s license to complete the process. When you finish the Registration process, print the confirmation page and give to your club director. You must also have completed the online Background Screening form (good for 2 seasons) and mail it to the Region Office. Note: The background screening process can not begin until your signed form and membership fee are in the Regional Office. All Chaperones need to sign the Chaperone Responsibility form and send to the Region Office.You will need to also do an Adult Medical Release form that you should carry to events. If you did not pay online, send your money and a copy of the confirmation to the Region Office or give to your Team Rep to send in as a group with other team members’ forms.
If you are an Adult member of a Junior Club or Team
You must have completed the online Background Screening form (good for 2 years and part of the online registration process) and mail to the Region Office. Note: The background screening process can not begin until your signed form and membership fee are in the Regional Office. You will need to also do an Adult Medical Release form that you should carry to events. If you did not pay online send your money and a copy of the confirmation to the Region Office or give to your Club Director to send in as a group with other team members’ forms.
If you are an Adult Player ONLY
Print your membership confirmation form (part of online registration process) and give it to your Team Rep. YYou will need to also do an Adult Medical Release form that you should carry to events. If you did not pay online send your money and a copy of the confirmation to the Region Office or give to your Team Rep to send in as a group with other team members’ forms.
If you are an Unaffiliated Member
If you did not pay online send your money and a copy of the confirmation to the Region Office.
- Be aware that with electronic acceptance it is important that adult members register themselves and that a parent/guardian registers a junior player.
- With the Online signature, the individual completing the registration for a junior player is representing that he/she has legal capacity and authority to act for and on behalf of the minor.
- Coaches and Club Directors should not be registering their players.It is reccommended to Club Directors to have parent's meeting in a setting where computers are available to register your their junior players on line. (Use a Computer Lab, Wireless Computer or at a Home with a computer.)
- To activate the Electronic Signature when you are online you need to open the two forms and then click the 3 acceptance boxes to complete the process.
- When using the Online Signature to verify who is filling out the form, the Adult or Parent/ Guardian will be asked for their Driver’s License or last 4 digits of SS # number, so have it on hand before starting the process.
- If you do not have a parent available for the online process you will need to do a paper membership form. The Junior player and parent will both have to sign the form and send to their Club director or the Iowa Region Office.
